Steps to Fill the New York Basic Rental Application
- Enter the date.
- Enter the application number.
- Enter the address of the premises.
- Enter the prospective tenant’s name.
- Indicate whether the prospective tenant is over 18 years old.
- Enter the prospective tenant’s Social Security number.
- Enter the prospective tenant’s home phone and work phone numbers.
- Indicate whether the prospective tenant has ever been convicted of a crime. Provide details if the answer is yes.
- Enter the prospective tenant’s current address, length of residence, monthly rent amount, and reason for leaving.
- Enter the prospective tenant’s current landlord’s name and phone number
- Enter the prospective tenant’s previous address, length of residence, monthly rent amount, and reason for leaving.
- Enter the prospective tenant’s previous landlord’s name and phone number
- Indicate if the prospective tenant has ever been evicted.
- Enter the names of any other proposed occupants.
- Indicate if there will be any pets, if there will be a waterbed, and if any of the prospective occupants smoke.
- Enter prospective tenant’s current employment information.
- Enter prospective tenant’s previous employment information.
- Enter prospective tenant’s vehicle information.
- Enter two personal references and emergency contact information for prospective tenant.
The prospective tenant must sign and date the application and attest that all information is complete and accurate. This gives the landlord or agent permission to perform a personal background check as necessary.
